Partnership for Hope

The Partnership for Hope is a creative program that more broadly serves and supports people in Missouri with development disabilities. The program is a unique collaboration between local county developmental disability boards, the Missouri Department of Mental Health and the federal Medicaid program. It is the first of its kind in the nation.

Eligibility Criteria

  • Person is eligible for Missouri Medicaid;
  • Person meets eligibility criteria for Division of DD services;
  • Person’s needs can be met with current community support system and waiver services not to exceed an annual cost of $12,000;
  • Meets ICF/ID Level of Care;
  • Person resides in a participating county; and
  • Person meets crisis or priority criteria.

Is there an age range eligible for the Partnership for Hope?
No. There is no age range for persons available for Partnership for Hope waiver.
What is the priority of need for Partnership for Hope?
Crisis
  1. Health and Safety conditions pose a serious risk of immediate harm or death to the individual or others;
  2. Loss of Primary Caregiver support or change in caregiver’s status to the extent the caregiver can’t meet needs of the individual; or
  3. Abuse, Neglect or Exploitation of the individual.

Priority

  1. Individual’s circumstances or conditions necessitate substantial accommodation that cannot be reasonably provided by the individual’s primary caregiver;
  2. Person has exhausted both educational and VR benefits or not eligible for VR benefits and have a need for pre-employment or employment services;
  3. Individual has been receiving supports from local funding for 3 months or more and services are still needed and the service can be covered by the waiver. Refinancing; or
  4. Person living in a non-Medicaid funded RCF chooses to transition to the community and determined capable of residing in a less restrictive environment with access to the PFH waiver.
Who determines the priority of need?
The participating County Board will prioritize the level of need based on the criteria spelled out in the question #2 above. County Boards that do not provide Targeted Case Management will coordinate the prioritization of enrollment in Partnership for Hope Waiver with the support coordinator.

The Productive Living Board for St. Louis County Citizens with Developmental Disabilities was established in 1979 when St. Louis County voters approved to tax themselves for the purpose of developing services and supports for residents with developmental disabilities.
